Enrollment Agreement

Eldorado Academy agrees to do the following:
1.   Request and evaluate the student's transcript and transfer credits;
2.    Assign and provide access to a pertinent curriculum;
3.    Assess and provide feedback of the student's work and suggest supplemental learning resources as needed;
4.    Generate documentation of academic progress upon completion of semester course work; and
5.    Grant an accredited High School diploma upon student completion of requirements.

Parent(s) and/or Sponsoring Adult(s) of the Student listed above agree to do the following:
1.    Pay a non-refundable registration fee of $100 and monthly tuition of $300 per student;
2.    Supervise the student's overall educational program and specific academic progress;
3.    Furnish computer hardware/software necessary to access Eldorado Academy courses/services;
4.    Provide supplemental learning resources as needed; and
5.    Monitor Internet sessions and student participation.

Student listed above agrees to do the following:
1.    Complete his/her own work according to the honor system;
2.    Document and report hours in non-academic courses of 45-hour increments to Eldorado Academy on the supplied form; and
3.    Show proper respect at all times to administrators, faculty, staff, and other students using appropriate language and tone.

Note: Submission constitutes acceptance of all terms of this contract. Eldorado Academy is a year-round program. Annual tuition may be paid in monthly installments for your convenience. Payment can be made by check, money order, automatic deduction (electronic funds transfer), Visa, and Mastercard. A service fee will be added to payments by credit card. The registration fee plus first month's tuition totaling $400 must accompany this enrollment form. Subsequent tuition payments of $300 per month are due in full at Eldorado Academy on the 1st day of each month regardless of enrollment date. Please send payments to the address listed on the Contact Us web page and make payable to "Eldorado Academy." Registration fees and tuition payments are non-refundable. Eldorado Academy reserves the right to assign late fees and/or suspend services without notice if any payment is not received in a full and timely manner. A student may withdrwaw from Eldorado Academy by notifying the school in writing by USPS or courier at least 30 days prior to the desired date of departure. A student may only re-enroll during the period of six months after the date of withdrawal at sole discretion of Eldorado Academy.
